研究目的：

（1）明确针刺脐周八穴调控PCOS蜕膜化进程，改善其妊娠结局的作用机制。 （2）为临床运用针刺脐周八穴提高PCOS活产率，改善其糖脂代谢水平提供科学依据。

Objectives of Study：

(1) To clarify the mechanism by which acupuncture at eight points around the umbilicus regulates the decidualization process of PCOS and improves its pregnancy outcome. (2) To provide a scientific basis for the clinical application of acupuncture at eight points around the umbilicus to increase the live birth rate of PCOS and improve its glycolipid metabolism level.

药物成份或治疗方案详述：

（1）预处理期治疗方案 1.针刺脐周八穴方案 ①针具：一次性针灸针φ0.25×40mm、安慰针φ0.25×25mm（华佗牌，苏州医疗用品厂有限公司）。 ②针灸师资质：至少为主治医师，并且具有至少2年的针灸临床经验。 ③针刺方案： 主穴：脐周八穴（水分、阴交、天枢、外陵、滑肉门） 配穴：子宫双侧、气海、关元、血海双侧、丰隆双侧、三阴交双侧 ④操作方法：嘱患者排空膀胱，平卧位，选取腹部腧穴水分、天枢、滑肉门、阴交、外陵、子宫直刺25~30mm，施捻转提插相结合泻法，施术1min；气海、关元直刺25~30mm，施捻转提插相结合补法，施术1min；三阴交、血海直刺30~35mm，施小幅度高频率捻转补法，施术1min；丰隆直刺30~35mm，施捻转提插泻法，施术1min。均留针30min。 2.安慰针刺方案 选取腹部及下肢的非经非穴点。 ①针具：安慰针φ0.25×25mm（华佗牌，苏州医疗用品厂有限公司）。 ②针刺方案： 刺激点1：脐上1寸，前正中线旁开6寸（足太阴脾经外侧2寸） 刺激点2：脐上2寸，前正中线旁开6寸（足太阴脾经外侧2寸） 刺激点3：脐下1寸，前正中线旁开6寸（足太阴脾经外侧2寸） 刺激点4：脐下2寸，前正中线旁开6寸（足太阴脾经外侧2寸） 刺激点5：脐下4寸，前正中线旁开6寸 刺激点6：屈膝在大腿内侧，髌底内侧端上3寸，股直肌肌腱内侧缘处。 刺激点7：内踝尖直上4寸，胫骨后缘旁开一横指（足太阴脾经外侧1寸） ③操作方法：嘱患者平卧位，针刺前将固定垫粘贴在穴位上。采用钝头针直刺穿过固定垫达皮肤表面但不刺破皮肤，不要求得气。每次月经周期结束后，均记录妊娠情况、针刺不良反应等。 （2）诱导排卵期治疗方案 诱导排卵周期自月经或地屈孕酮撤退出血第3天开始服用来曲唑（江苏恒瑞医药股份有限公司生产，商品名：芙瑞），每2.5mg，连续服用5天，自月经第8天开始监测卵泡。若当周期有优势卵泡发育，卵泡直径≥18mm时，使用HCG5000-10000IU肌肉注射。若排卵未妊娠，于下次月经第3天重复上述治疗，常规治疗20周，进行3-5个周期；若当周期未有优势卵泡发育，28天后开始服用来曲唑，来曲唑剂量增加2.5mg，最高剂量不超过7.5mg。诱导排卵期进行卵泡监测，并指导患者同房。剂量可根据反应进行调整，或在确定了适当的反应时予以维持。来曲唑的最大剂量不超过37.5mg/周期。

Description for medicine or protocol of treatment in detail：

(1) Pretreatment treatment plan 1. Needling the eight acupoints around the umbilicus ① Needles: Disposable acupuncture needles φ0.25×40mm placebo needles φ0.25×25mm (Hua Tuo brand Suzhou Medical Supplies Factory Co. LTD.). ② Acupuncturist qualifications: At least a chief physician and with at least 2 years of clinical experience in acupuncture. ③ Acupuncture plan: Main acupoints: Eight acupoints around the umbilical cord (Shui Shui Yin Jiao Tian Shu Wai Ling Hua Rou Men) Acupoints selected: Both sides of the uterus Qihai Guanyuan both sides of Xuehai both sides of Fenglong both sides of Sanyinjiao ④ Operation method: Instruct the patient to empty the bladder and lie flat. Select abdominal acupoints such as Shuihui Tianshu Huaruomen Yinjiao Wailing and Uterus and directly puncture them 25 to 30mm. Apply a combined method of twisting rotating lifting and inserting to purge the patient and perform the operation for 1 minute. Puncture the Qihai and Guanyuan straight for 25 to 30mm apply a combined method of twisting turning lifting and inserting for supplementation and perform the operation for 1 minute. Puncture the Sanyinjiao and Xuehai points 30 to 35mm directly apply a small-amplitude and high-frequency twisting and tonifying method and perform the operation for 1 minute. Insert the Fenglong straight puncture 30 to 35mm apply the twisting turning lifting thrusting and draining method and perform the operation for 1 minute. All were retained for 30 minutes. 2. Placebo acupuncture regimen Select non-meridian and non-point points on the abdomen and lower limbs. ① Needle: Placebo needle φ0.25×25mm (Hua Tuo brand Suzhou Medical Supplies Factory Co. LTD.). ② Acupuncture plan: Stimulus point 1:1 cun above the umbilicus 6 cun lateral to the anterior midline (2 cun lateral to the spleen meridian of Foot-Taiyin) Stimulus point 2:2 cun above the umbilicus 6 cun lateral to the anterior midline (2 cun outside the spleen Meridian of Foot-Taiyin) Stimulus point 3:1 cun below the umbilicus 6 cun lateral to the anterior midline (2 cun lateral to the spleen meridian of Foot-Taiyin) Stimulus point 4:2 cun below the umbilicus 6 cun lateral to the anterior midline (2 cun outside the Spleen Meridian of Foot-Taiyin) Stimulation point 5:4 cun below the umbilicus 6 cun lateral to the anterior midline Stimulus point 6: Bend the knee on the inner side of the thigh 3 cun above the inner end of the patellar base at the inner edge of the rectus femoris tendon. Stimulus Point 7:4 cun directly above the tip of the medial malleolus one transverse finger to the side of the posterior edge of the tibia (1 cun outside the Spleen Meridian of Foot Taiyin) ③ Operation method: Instruct the patient to lie flat. Before acupuncture attach the fixed pad to the acupoint. Use a blunt-tipped needle to directly Pierce through the fixed pad to reach the skin surface without piercing the skin and do not seek air. After each menstrual cycle ended the pregnancy status adverse reactions of acupuncture etc. were recorded. (2) Ovulation induction treatment plan The ovulation induction cycle starts with letrozole (produced by Jiangsu Hengrui Medicine Co. LTD. trade name: Furui) on the third day after the withdrawal of bleeding from dydrogesterone or menstruation at a dose of 2.5mg for 5 consecutive days. Follicles are monitored starting from the eighth day of menstruation. If there is a dominant follicle developing in the cycle and the follicle diameter is ≥18mm HCG5000-10000IU should be injected intramuscularly. If ovulation does not lead to pregnancy repeat the above treatment on the third day of the next menstrual cycle. The conventional treatment lasts for 20 weeks and is carried out for 3 to 5 cycles. If no dominant follicle develops in the current cycle letrozole can be taken 28 days later. The dose of letrozole should be increased by 2.5mg and the maximum dose should not exceed 7.5mg. During the ovulation induction period follicular monitoring is conducted and the patient is guided to have sexual intercourse. The dosage can be adjusted according to the reaction or maintained when an appropriate reaction is determined. The maximum dose of letrozole does not exceed 37.5mg per cycle.

纳入标准：

1.年龄20-40岁； 2.符合PCOS诊断标准； 3.符合胰岛素抵抗诊断标准； 4.在过去3年内输卵管通畅试验，显示至少有一侧输卵管通畅； 5.丈夫的精液分析正常。 6.受试者自愿参加并签署知情同意书。

Inclusion criteria

1. Age: 20-40 years old; 2. Meet the diagnostic criteria for PCOS; 3. Meet the diagnostic criteria for insulin resistance; 4. The fallopian tube patency test in the past three years shows that at least one fallopian tube is unobstructed. 5. The husband's semen analysis is normal. 6. The subjects voluntarily participated and signed the informed consent form.

排除标准：

1.合并其他生殖系统疾病。 2.合并其他内分泌疾病。 3.合并内科、外科、精神科等严重原发疾病者。 4.使用过其他已知会影响生殖功能或代谢水平药物的患者。 5.目前有酗酒史的患者。 6.最近3个月内经历过药物减重或节食减重。 7.最近3个月针对本病接受过针刺治疗。 8.近6周内妊娠、流产史、生育史。 9.近6个月内哺乳。 10.存在其它影响妊娠的疾病。

Exclusion criteria：

1. Combined with other reproductive system diseases. 2. Concurrent with other endocrine diseases. 3. Those with serious primary diseases in internal medicine surgery psychiatry etc. 4. Patients who have used other drugs known to affect reproductive function or metabolic levels. 5. Patients who currently have a history of alcohol abuse. 6. Have experienced weight loss through medication or dieting within the last three months. 7. Acupuncture treatment for this disease has been received in the last three months. 8. History of pregnancy miscarriage and childbirth within the last 6 weeks. 9. Breastfeeding within the last six months. 10. There are other diseases that affect pregnancy.
