研究目的：

1. 主要目的：从微生物学、代谢组学及分子生物学三个角度科学阐述慢性荨麻疹胃肠湿热证的分子机制，探索胃肠道微生态失衡导致CU胃肠湿热证的病理机制。 2. 次要目的：研究慢性荨麻疹中医证型的分子机制，为提高慢性荨麻疹的临床疗效提供更加客观的依据。

Objectives of Study：

1. Main objective: To scientifically explain the molecular mechanism of of chronic urticaria with syndrome of dampness and heat in the intestines and stomach from the perspectives of microbiology, metabolomics and molecular biology, and to explore the pathological mechanism of gastrointestinal microecological imbalance leading to chronic urticaria with syndrome of dampness and heat in the intestines and stomach. 2. Secondary objective: To study the molecular mechanism of TCM syndrome type of chronic urticaria and provide a more objective basis for improving the clinical efficacy of chronic urticaria.

Description for medicine or protocol of treatment in detail：

纳入标准：

1.慢性荨麻疹患者纳入标准：①年龄18-50岁；②符合慢性荨麻疹的西医诊断标准；③符合中医瘾疹的诊断标准；④辨证符合瘾疹胃肠湿热证或非胃肠湿热证（风热证、风寒证、毒热炽盛证或气血亏虚证）；⑤签署知情同意。 2.健康受试者纳入标准：①年龄18-50岁；②近１年无荨麻疹和胃部疾病史；③C13呼气试验或抗HP抗体阴性；④近3个月未进行抗生素、激素、免疫抑制剂、生物制剂治疗，或近1周未接受中药或中成药治疗；⑤签署知情同意。

Inclusion criteria

1. Inclusion criteria for patients with chronic urticaria: ① Age 18-50; ② Meet the western diagnostic criteria of chronic urticaria; ③ Meet the diagnostic criteria of TCM for chronic urticaria; ④ TCM syndrome differentiation conforms to chronic urticaria gastrointestinal dampness and heat syndrome or not syndrome of dampness and heat in the intestines and stomach (wind-heat syndrome, wind-cold syndrome, toxic heat and excessive heat syndrome or qi and blood deficiency syndrome); ⑤ Sign informed consent. 2. Inclusion criteria for healthy subjects: ① Age 18-50; ② No history of urticaria and stomach diseases in the past 1 year; ③ Negative C13 breath test or anti-HP antibody; ④ No antibiotics, hormones, immunosuppressants, or biological agents in the last 3 months, or no Traditional Chinese medicine or patent Chinese medicine in the last 1 week; ⑤ Sign informed consent.

排除标准：

慢性荨麻疹患者排除标准：①患有急性荨麻疹、药物性荨麻疹、诱导性荨麻疹（冷接触性荨麻疹、热接触性荨麻疹、日光性荨麻疹、延迟压力性荨麻疹、震动性荨麻疹、胆碱能性荨麻疹、接触性荨麻疹等）、荨麻疹性血管炎及其他伴有荨麻疹样表现的皮肤疾病；②近3个月接受过激素、免疫抑制剂、生物制剂治疗，或入组前1周接受过相关中药治疗；③妊娠、准备妊娠或哺乳期妇女；④合并严重呼吸系统、消化系统、循环系统、泌尿系统、血液系统、内分泌系统或神经系统等疾病史。⑤具有以下影响粪便留取的因素：（1）5年内进行过胃肠道手术（如：胆囊切除术和阑尾切除术），或6个月内进行过其他手术；（2）既往有自身免疫病病史，如：炎症性肠病（IBD），包括溃疡性结肠炎、克罗恩病或病因不明的结肠炎；（3）既往有肠易激综合症（IBS）、胃癌以及胃癌前病变病史；（4）3个月内使用过抗生素；（5）近1周内使用过以下药物：含碱式水杨酸铋和其它类似的铋碱式水杨酸盐成分的药物；（6）近2周内进进食过含益生菌的食物或药物，如酸奶、奶酪等；（7）有酗酒等不良嗜好，男性一周饮酒5次及以上，女性一周饮酒4次及以上，平均每次饮酒量多于2两白酒或5瓶啤酒。⑥拒绝签署知情同意书的患者。 健康受试者排除标准：①妊娠、准备妊娠或哺乳期妇女；②存在严重呼吸系统、消化系统、循环系统、泌尿系统、血液系统、内分泌系统或神经系统等疾病史；③近3个月内有急性感染；④有过敏性疾病史或相关过敏性症状；⑤近1个月内有任何已知的用药史。⑥具有以下影响粪便留取的因素：（1）5年内进行过胃肠道手术（如：胆囊切除术和阑尾切除术），或6个月内进行过其他手术；（2）既往有自身免疫病病史，如：炎症性肠病（IBD），包括溃疡性结肠炎、克罗恩病或病因不明的结肠炎；（3）既往有肠易激综合症（IBS）、胃癌以及胃癌前病变病史；（4）3个月内使用过抗生素；（5）近1周内使用过以下药物：含碱式水杨酸铋和其它类似的铋碱式水杨酸盐成分的药物；（6）近2周内进食过含益生菌的食物或药物，如酸奶、奶酪等；（7）有酗酒等不良嗜好，男性一周饮酒5次及以上，女性一周饮酒4次及以上，平均每次饮酒量多于2两白酒或5瓶啤酒。⑦拒绝签署知情同意书的患者。

Exclusion criteria：

1.Exclusion criteria for patients with chronic urticaria: (1) with acute urticaria, drug sex urticaria, induced urticaria (cold sex urticaria, hot sex urticaria, solar sex urticaria, delay pressure sex urticaria, anti-vibration effect sex urticaria, cholinergic urticaria, sex urticaria, etc.), urticaria, vasculitis and other associated with urticaria "appearance of the skin disease; ② Had received hormone, immunosuppressant or biological agent treatment in the last 3 months, or received relevant Traditional Chinese medicine treatment one week before enrollment; ③ Women who are pregnant, trying to become pregnant or breastfeeding; ④ With a history of severe respiratory, digestive, circulatory, urinary, hematological, endocrine or nervous system diseases. ⑤ There were the following factors affecting fecal collection: (1) gastrointestinal surgery (e.g., cholecystectomy and appendectomy) within 5 years, or other operations within 6 months; (2) a history of autoimmune disease, such as inflammatory bowel disease (IBD), including ulcerative colitis, Crohn's disease, or colitis of unknown etiology; (3) Previous history of irritable bowel syndrome (IBS), gastric cancer and pregastric cancer; (4) Used antibiotics within 3 months; (5) drugs containing bismuth basic salicylate and other similar bismuth basic salicylate have been used in the past 1 week; (6) Have eaten probiotic-containing food or drugs, such as yogurt and cheese, in the past 2 weeks; (7) Have bad habits such as alcoholism, men drink 5 times a week or more, women drink 4 times a week or more, the average amount of alcohol per time is more than 2 liang liquor or 5 bottles of beer. ⑥ Patients who refuse to sign informed consent. 2.Exclusion criteria for healthy subjects: ① women who are pregnant, trying to become pregnant or breastfeeding; ② There is a history of severe respiratory, digestive, circulatory, urinary, hematological, endocrine or nervous system diseases; ③ Acute infection within the last 3 months; ④ Have a history of allergic diseases or related allergic symptoms; ⑤ Any known medication history within the last 1 month. 数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

我们使用病例报告表(CRF)进行数据的采集，使用Epi Info软件进行数据的管理。根据CRF表格的项目采用软件建立本试验专用的数据库系统，数据管理员对病例报告表进行检查及数据同步录入，数据的审核先用采用Epi Info软件中的核查功能，再随机抽取10份病例报告表和数据库中的数据进行人工比较，以确保数据库中的数据与CRF中的结果一致。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

We use the case report form for data collection, and use Epi Info software for data management. According to the items in the CRF form, the software is used to establish a special database system for this experiment. The data manager checks the case report form and synchronizes data entry and data review. We first adopt the verification function in Epi Info software to review the data, and then randomly select 10 case report forms and data in the database for manual comparison to ensure that the data in the database is consistent with the results in the CRF.
