研究目的：

本研究旨在对早产儿采取不同体位，利用近红外广谱测定技术监测不同体位下的早产儿脑组织氧饱和度，为临床评价脑组织氧合状况提供依据，以期指导临床护理人员对早产儿开展有效的体位管理和病情监测，这对于促进早产儿舒适，减轻疾病的痛苦，预防并发症，促进患儿尽早康复具有非常重要意义。

Objectives of Study：

Therefore, this study aimed to use NIRS to monitor the regional cerebral oxygen saturation of premature infants under different body position. The results could provide some basis for the clinical evaluation of brain tissue oxygenation status and to guide the effective position management to promote comfort, to alleviate pain, to prevent complications, and to promote the early recovery of premature infants.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

1）符合早产儿的诊断标准；2）新生儿出生1min、5min Apgar评分≧8分。

Inclusion criteria

1) premature infants; 2) 10 min and 5-min Apgar scores >8 points

排除标准：

需呼吸机辅助呼吸、新生儿窒息复苏后、先天性发育异常、先天性心脏病、胃肠道功能异常、新生儿颅内出血、重度缺氧缺血性脑病、贫血和出血性疾病、败血症、坏死性小肠结肠炎等。

Exclusion criteria：

1) ventilator-assisted breathing; 2) neonatal asphyxia resuscitation; 3) congenital developmental abnormalities; 4) congenital heart disease; 5) gastrointestinal dysfunction; 6) neonatal intracranial hemorrhage; 7) severe hypoxic-ischemic encephalopathy; 8) anemia and hemorrhagic diseases; 9) sepsis; 10) necrotizing enterocolitis.

从住院后第2天开始，持续一周，每天从09:00开始，为患儿按照（15°俯卧位、15°仰卧位、0°俯卧位、0°仰卧位）顺序依次摆放体位；以2-3个小时为一个喂奶周期，每个喂奶周期摆放一个体位；所有患儿在喂养时置于仰卧位，头部抬高15°～30°稍倾斜向一侧，喂养后 15 min 置于相应体位，除治疗、喂奶、皮肤护理等时间外，均保持相应体位，直至下个喂奶周期。体位摆放后应检查各管道固定是否良好，避免管道受压，保持床单整洁、透气，防止发生压伤、擦伤等皮肤完整性受损，男性患儿避免外生殖器受压。
