研究目的:

魏氏伤科是上海地区较有影响力的中医骨伤科流派,其治疗膝骨关节炎以手法 为特色。在前期临床研究中,已经证实魏氏手法可有效缓解疼痛、关节活动障 碍等症状,有较好的临床疗效。功能磁共振自旋锁定成像(T1ρ)是近年来发 展起来的新技术,其不但可以显示骨骼、半月板、滑膜等组织结构变化,更能 从生化结构上定量评估软骨基质中胶原含量、软骨细胞排列和水含量变化,是 发现早期骨关节炎的有效方法。功能磁共振经过十余年的发展,已具备了从实 验室走向临床的条件,但如何优化功能磁共振成像技术参数,建立适应我国临 床实际情况,尤其建立适合评估膝骨关节炎疗效的序列组合,则是临床与影像 工作者所必须应对的挑战。本研究对魏氏伤科手法治疗膝骨关节炎随机对照研 究进行临床数据挖掘,建立以磁共振 T1ρ成像技术为核心的膝骨关节炎疗效 评价系统,以此系统为基础探讨魏氏伤科手法对膝骨关节炎疗效,以自然科学 界所广泛接受的技术语言重新阐述古老的中医手法技术。

Objectives of Study:

Wei's traumatology is an influential traditional Chinese medicine in Shanghai. In the early clinical research, it has been confirmed that Wei's manipulation can effectively relieve pain, joint movement disorders and other symptoms, and has good clinical effect. Functional magnetic resonance spin lock-in imaging (T1 ρ) is a new technology developed in recent years. It can not only show the changes of bone, meniscus and synovium, but also quantitatively evaluate the collagen content, chondrocyte arrangement and water content changes in cartilage matrix from the biochemical structure. It is an effective method to detect early osteoarthritis. After more than ten years of development, functional magnetic resonance imaging has the conditions to move from laboratory to clinical. However, how to optimize the technical parameters of MRI, establish the sequence combination suitable for the clinical situation in China, especially to establish the sequence combination suitable for evaluating the curative effect of knee osteoarthritis, are the challenges that clinical and imaging workers must face. In this study, the clinical data mining of Wei's traumatology manipulation in the treatment of knee osteoarthritis was carried out, and the therapeutic effect evaluation system of knee osteoarthritis with magnetic resonance T1 ρ imaging technology as the core was established. Based on this system, the curative effect of Wei's traumatology manipulation on knee osteoarthritis was discussed, and the ancient Chinese medicine manipulation technology was reinterpreted in the technical language widely accepted by the natural science community.

纳入标准:

①符合上述美国风湿病协会的诊断标准; ②年龄40~70岁; ③根据患者X线影像,采用Kellgren/Lawrence分级[5]为I级~III级的患者; ④同意参与本项研究,签署知情同意书。

Inclusion criteria

1. According to the diagnostic criteria of the American rheumatic Association; 2. Aged 40-70 years; 3. Patients with Kellgren / Lawrence classification I-III according to the X-ray images of the patients; 4. They agreed to participate in the study and sign the informed consent.

排除标准:

① 膝关节急性外伤、骨结核、肿瘤、关节感染、类风湿关节炎等疾病患者; ② 合并呼吸道、严重心脑血管、精神等内科疾病; ③ 患者装有心脏起搏器,有金属植入物,以及其他疾病无法配合MR检查患者。

Exclusion criteria:

1. Patients with acute knee trauma, bone tuberculosis, tumor, joint infection, rheumatoid arthritis and other diseases; 2. Patients with respiratory tract, severe cardiovascular and cerebrovascular diseases, psychiatric diseases; 3. patients with cardiac pacemaker, metal implants, and other diseases unable to cooperate with MR examination.
